Accepting new participants across South Australia

Support coordination that gets things moving.

Your NDIS plan is only useful once it turns into real supports, in your real life. Adaptive Innovations helps South Australians make sense of their plan, connect with providers worth keeping, and build the confidence to take the lead over time.

The plain version

What is support coordination?

A support coordinator is the person who helps you turn the words in your NDIS plan into supports that actually show up. We explain what your funding can and can't do, find providers who suit you, sort out the paperwork, and step in when something isn't working.

The goal isn't for you to need us forever. It's for you to know your plan well enough to run it your way.

We do

Explain your plan, find and vet providers, set up service agreements, track your budget, prepare you for plan reviews, and coordinate everyone involved.

We don't

Push you toward services we own or profit from, make decisions for you, or disappear between reviews.

Not sure which level you're funded for?

Send us your plan and we'll read it and tell you — free, no obligation.

What we offer

Four ways we can work together

See full service detail
L2

Coordination of Supports

The everyday work: understanding your plan, choosing providers, getting services started and keeping them on track.

L3

Specialist Support Coordination

For high-risk or complex situations — hospital discharge, housing breakdown, multiple agencies pulling in different directions.

PRC

Psychosocial Recovery Coaching

Longer-term, recovery-focused support for people living with mental health conditions, built around good days and hard ones.

PM

Plan Management

Invoices paid on time, budgets tracked, and a clear monthly picture of where your funding has gone.

How it works

Getting started takes one phone call

STEP 1

Get in touch

Call, email or send your plan through the form. We'll confirm your funding covers coordination.

STEP 2

Meet and map it out

We sit down — your place, a café, or online — and go through your plan, your goals and what's not working.

STEP 3

Build your team

We shortlist providers, arrange meet-and-greets, and set up agreements so services start properly.

STEP 4

Stay in your corner

Regular check-ins, budget tracking, and a report ready well before your next plan review.

Questions we get asked a lot

Look in the Capacity Building section of your plan for 'Support Coordination' or 'Specialist Support Coordination'. If you're not sure, send us your plan and we'll read it and tell you — free, and with no expectation you sign up.

No. It's paid from the coordination funding already in your NDIS plan, at NDIS price-guide rates. It doesn't come out of your other supports.

Yes. You can change support coordinators at any time — you don't need a reason and you don't need to wait for a plan review. We'll handle the handover so nothing falls through the cracks.

We support participants across South Australia. Where travel isn't practical we work by phone and video, and we can meet in person for the moments that matter most.

Usually within a few days of your enquiry. We'll confirm your funding, send a service agreement, and book a first meeting at a place that suits you.

Absolutely. Send the referral through the enquiry form with as much context as you can, and we'll come back to you within one business day.

Ready when you are.

Send your plan through and we'll tell you what it covers — no obligation.
